The sense of waiting for the next seizure surfaced repeatedly in patients' own words. One respondent described being "always worried when or if I'll have another seizure," and another wrote of feeling like there was "a dormant monster waiting to strike at any moment.”1
This burden is not limited to patients with frequent seizures. Just under half of respondents strongly agreed that their epilepsy was well-controlled on their current treatment, and 60 percent had experienced a seizure within the past year – leaving many patients living with ongoing uncertainty about when the next seizure may occur. For these patients, the uncertainty surrounding future seizures can become a chronic source of stress, even between seizures.1
What unpredictability takes away
The impact extends well beyond seizure frequency. Asked how much epilepsy negatively affects specific areas of life, respondents rated employment or education among the most affected (40 percent), followed closely by their sense of independence (38 percent). Mental and emotional health was rated as significantly affected by a comparable share (35 percent), and overall quality of life was rated fair or poor by 42 percent.1
Driving restrictions were a recurring theme in the open-ended responses, with patients describing the negative effects on employment, social engagement, and independence. One respondent captured the isolation directly, writing that being "scared to leave the house alone" made it hard to "make plans or keep commitments.”1
Taken together, these responses suggest that the uncertainty surrounding future seizures influences decisions about work, transportation, social activities, and independence in ways that may not be reflected by seizure frequency alone.

The burden of seizure unpredictability is not always proportional to seizure frequency. Even patients with relatively infrequent seizures may substantially restrict driving, employment, travel, or social activities because of uncertainty about when the next seizure will occur.
Because the impact of seizure unpredictability does not appear in a seizure count, it is easy for it to go unaddressed. Patients may not volunteer anticipatory anxiety or the practical losses it drives unless asked, so a brief, direct question about how the possibility of a seizure affects daily decisions can surface issues that may not be apparent from seizure frequency alone.
Asking specifically about driving, work, changes in daily activities, and whether the patient avoids being alone often elicits more than a general question about mood, because these conversations often reveal functional limitations that would otherwise go unrecognized. From there, the clinical response is tangible: For patients who are appropriate candidates, an individualized acute plan gives patients and care partners a defined action to take when a seizure or cluster begins, which reframes the next event from something that simply happens to something the patient is prepared to meet.
Although an acute plan cannot eliminate the uncertainty of epilepsy, it can give patients and caregivers a clear strategy for responding to breakthrough seizures or seizure clusters, which many patients find reassuring. Treating seizure unpredictability as an important part of epilepsy care aligns the encounter with many of the challenges patients identify as having the greatest impact on daily life.
